About end-of-life care

Caring for someone at the end of life is one of the most important things a family can do, and one of the hardest. Our aim is to make this time as peaceful and meaningful as possible, with care that is guided by what matters most to the person and the people closest to them.

We take time to listen, so we understand their wishes, routines, beliefs and preferences, and we shape the care plan around them. Our carers are patient, respectful and unhurried, and our nurse-led approach means clinical knowledge sits behind every visit.

Medical decisions and medicines are directed by the person's own doctors and nurses. We work alongside them, follow the plan they set, and let the right people know promptly if something changes.

What our end-of-life care can include

Gentle personal care that keeps your loved one clean, comfortable and dignified
Companionship, reassurance, and time to talk or sit quietly together
Help with eating and drinking, in line with their wishes and comfort
Care through the night, so families can rest knowing someone is there
Working alongside GPs, district nurses and community palliative care teams
Kindness and practical support for family members

Who it can help

People with a life-limiting illness who wish to be cared for at home
Families who want to be present, supported and not alone
People returning home from hospital or a hospice for their final weeks
People whose needs are changing and whose care plan needs to change with them

Frequently asked questions

Can you care for someone who wants to stay at home at the end of life?
Many people want to, and we will talk honestly with you about what we can provide, how we would work with the doctors and nurses involved, and whether any extra support may be needed.
How quickly can care start?
Call us as soon as you can. We will tell you honestly how soon we can help and agree a plan with you.
Is funding available for end-of-life care?
Some people qualify for NHS Continuing Healthcare, which includes a fast track route for people with a rapidly worsening condition. Their care team can advise on eligibility, and we are happy to talk through the options when you call for a free assessment.
